Responsibilities
Mayo Clinic’s Departments of Pharmacy and Nephrology are seeking a dedicated and skilled Nephrology Ambulatory Care Pharmacist to join our Medication Management Services (MMS) team. This 0.8 FTE position will be based out of Rochester, MN, with flexible working hours that may vary between 7 a.m. – 4 p.m. and 8 a.m. – 5 p.m., Monday through Friday (designated day off to be determined).
In this role, the pharmacist will build a practice within the Nephrology Department, focusing on comprehensive medication management of complex patients and optimizing the use of guideline-directed, complex therapy. This positions FTE will be split within the nephrology department: 0.3 FTE will provide ongoing care to dialysis patients at Eisenberg, Northeast and home dialysis units completing comprehensive reviews; the additional 0.5 FTE will be used for expansion to develop a new practice within the general nephrology clinic.
The primary responsibilities include providing direct patient care (both in-clinic and virtual visits) and indirect patient care (supporting medication therapy processes, assisting with patient, provider and care team inquiries). This position also offers opportunities for administrative tasks and process improvement activities related to patient care.
In addition to clinical responsibilities, the pharmacist will actively contribute to quality improvement initiatives, research efforts, and education programs. You will work closely with a diverse team, including physicians, advanced practice providers, nurses, and other clinical staff, to ensure optimal pharmacotherapy for patients.
Key Responsibilities:
Conduct patient-centered medication management patient visits.
Work collaboratively with care team to optimize medication use and patient outcomes.
Document and complete billing processes for pharmacist activities.
Collaborate in protocol-driven practices and/or collaborative practice agreements as applicable.
Contribute to quality improvement and resource management projects.
Support research initiatives within the department and clinics.
Participate in precepting activities for students and residents.
Contribute to continuous improvement and patient safety efforts.
Assist with other departmental projects and initiatives.
Collaborate with Mayo Clinic Specialty Pharmacy, Infusion Therapy Center, and insurance prior authorization teams.
Qualifications
Required Qualifications
- Education/Experience: Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program OR Bachelor of Science (B.S.) AND 2-years relevant experience.
- Licensure: Minnesota Pharmacist license
Preferred Qualifications
- Education/Experience: If holding a Bachelor's degree, 10-years of relevant experience is preferred.
- Medication Therapy Management (MTM): If limited experience in Medication Management Ambulatory Care, completion of an accredited MTM certificate program is required within 12-months of hire.
- Minnesota Health Care Programs (MHCP): Must be registered or eligible to be a registered MMS/MTM provider through Minnesota Health Care Programs (MHCP).
- Ambulatory Care Experience: Experience in Ambulatory Care practice, especially with collaborative practice agreements (CPAs).
- PGY-2 Pharmacy Residency: Completion of an accredited PGY-2 pharmacy residency in ambulatory care.
- Nephrology Care Experience: Experience providing care to patients with chronic kidney disease and end stage renal disease (dialysis).
- Board Certification (BCACP): Board certification in Ambulatory Care Pharmacy (BCACP) is preferred.
- Skills and Experience in Practice and Practice Development: Prior involvement in practice initiation, development, community service, research, quality improvement, and leadership roles.
- Ideal candidates demonstrate excellence in collaboration, teamwork, flexibility and are prepared to excel in delivering meaningful resultsand outcomes that are strategically aligned with the organization’s objectiveswhile focusing on continuous quality and performance improvement. Success in this position will require candidates to be engaged and proactive with practice development and optimization.
- Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations preferred.
- Commitment to excellence, accuracy, attention to detail and teamwork required.
- Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.
